Warranty Information
AZ Dental Esthetics is committed to providing exceptional products and services to our customers. To ensure that our customers are pleased with out work we have a one year guarantee on all our completed cases from the date of invoice, with the exception of minimal or no prep veneers which do not carry a warranty.
What Is Not Covered
– Costs incurred from removal or reinsertion (including inconvenience, lost chair time, lost wages or pain and suffering)
– Product or shade change different from the original prescription
– The Lab inquiring about the impressions, margins, die or photos. Dentist approved and requested to go to completion on case
– Prosthesis returned to us that show alterations by others, abuse, or misuse
– Interim prosthesis (6 month warranty from date of invoice)
Returns
– Returns must be completed within 14 days
Terms
We will reuire a credit card on file. For your convenience, we accept VISA, MASTERCARD, and AMERICAN EXPRESS. Payments not received within 30 days of statement date are subject to a 2% per month service charge. We will automatically process your credit card on account balances exceeding 45 days. In the event of a declined charge, you will be placed on C.O.D. until further notice. All costs for collection on any account will be the responsibility of the debtor including attorney fees. Prices subject to change without notice.
Customer Agreement
– Please simply return the orignal case with model work and we will adjust or repair the case at no charge
– There are no refunds on any fabricated custom dental prosthesis
– If a cancellation of a case occurs during the fabrication process we would charge you the cost incurred to that point. No refunds will be issues, only lab credit
